Just complete mine :) Actually I just get some idea baout this when attending Kursus Pra Perkahwinan few months back. But by the time kena isi borang2 ni..haha, pening jugak. Borang ni boleh diambil dari pejabat agama islam daerah(PAID) atau atau muat turun dari laman web JAIS(kat informative links). Borang pemeriksaan HIV boleh dibeli di PAID. They will provide us with list of Klinik Kerajaan that we can go for HIV test. This test result only valid for 6 months. Then you may start to fill up borang permohonan kahwin. For gals, make sure your FH settle their forms first! Then baru la isi, tandatangan borang bersama wali n 2 org saksi kat qarriah tempat tinggal. Once completed, you may submit the complete form to PAID. Contohnya yg tinggal kat Pandan Indah, qarriah ialah Masjid Al Azim. Since ia di bawah daerah Hulu Langat, maka borang yg telah lengkap hendaklah dihantar ke PAID Hulu Langat. Tetapi sekiranya bercadang untuk nikah di luar daerah(e.g. Gombak instead of Pandan Indah), applcant must get approval from PAID Hulu Langat then only submit the form to particular PAID in this case Gombak.
